This role focuses on delivering person-and family-centered care to support those with diverse backgrounds. As a critical member of the multi-disciplinary team, you will implement behavioral interventions, teach activities of daily living (ADLs), and collaborate on individualized care plans. Key Responsibilities:
• Implement behavioral and rehabilitation interventions for community patients
• Teach ADLs and apply relevant OT, PT, and SLP strategies




• Provide routine care and escort clients to appointments
• Conduct assessments and document patient progress
• Collaborate with healthcare specialists for comprehensive patient support

Requirements:
• Minimum 2 years experience in behavior assessments
• Post-secondary education in behavior sciences or psychology
• Knowledge of psychogeriatric assessment tools
• Robust communication and problem-solving skills
• Ability to manage intense emotional situations
