As a Float Care Coordinator, you will assess patient needs, create service plans, and connect clients to community resources. Key Responsibilities:
• Assess and engage clients for effective service planning
• Perform multi-dimensional patient assessments and identify crisis needs
• Develop and implement personalized care plans with healthcare teams
• Monitor and reassess care plans to meet quality standards
• Manage discharge planning and resource referrals
Requirements:
• Registered nurse or allied health qualified with relevant degree
• Minimum of 2 years qualified experience in related fields
• Understanding of healthcare systems and community resources
• Fluency in English; French is an asset
• Proof of COVID-19 vaccination required
