Contribute to community health at St. Joseph’s Healthcare Hamilton as a Peer Support Provider within the Integrated Care Pathway. Engage with individuals in the Emergency Department and promote vital connections to care and services.
This part time, temporary role focuses on delivering both informal and formal peer support to individuals facing substance use challenges. Collaborating with a multidisciplinary team, the Peer Support Provider will facilitate access to addiction treatment, harm reduction strategies, and essential community resources. Your lived experience and understanding of peer support values will make a significant impact on patients' lives.
Key Responsibilities:
• Provide peer support within the Emergency Department
• Assist individuals in navigating treatment and community services
• Offer education on harm reduction and substance use options
• Collaborate with healthcare providers to ensure seamless transitions
• Foster a respectful and non-judgmental environment for patients
Requirements:
• Minimum one year of peer support experience
• Personal lived experience with substance use preferred
• Knowledge of addiction services and harm reduction strategies
• Robust communication and interpersonal skills
• Commitment to continuous learning and career development
