Become a key player in healthcare as the Manager of Primary Care Transformation at Flemingdon Health Centre. This full-time position requires robust project management skills and engagement strategies in a community health setting.

As a Manager, you will report to the Director of Primary Care Transformation and play a vital role in leading projects that enhance primary care access. Your responsibilities will include overseeing implementation strategies for Interprofessional Primary Care Teams, managing competing priorities, and producing clear communication materials.

Key Responsibilities:
• Lead PCN/IPCT projects through planning and execution
• Maintain detailed project workplans and logs
• Prepare comprehensive project documentation for stakeholders




• Coordinate with partners to advance healthcare priorities
• Plan and support PCN working group meetings

Requirements:
• Undergraduate degree in Health Administration or related field
• Minimum four years’ project management experience
• Robust communication skills for preparing explicit materials
• Ability to manage multiple stakeholders effectively
• Experience in healthcare or community health initiatives
