You will utilize your experience in social services and community health, ensuring participants gain access to shelter resources, healthcare, and social services. Key Responsibilities:
• Conduct direct outreach activities in East Portland
• Assist participants in accessing healthcare and shelter services




• Maintain participant charts with effective notes and outcomes
• Provide basic life skills education and support
• Adhere to privacy regulations and documentation standards

Requirements:
• High school diploma or GED required
• 1 year of experience in human services or community health
• Valid driver’s license and CPR certification
• Understanding of homelessness complexities
• Solid communication and relationship-building skills
