Support older adults facing mental health challenges as an Older Adult Outreach Nurse (RN/RPN) with Vancouver Coastal Health in Vancouver, BC. This position is vital for delivering compassionate care to those at risk due to aging-related issues.
As a registered nurse in this role, you'll engage with older adults suffering from dementia and major psychiatric disorders. Your responsibilities include conducting assessments, crisis intervention, and facilitating individual and group therapy sessions. Key Responsibilities:
• Perform psychogeriatric assessments and interventions
• Facilitate case coordination and treatment planning
• Provide psycho-education and counseling to clients

Requirements:
• Registered Nurse or Registered Psychiatric Nurse (BCCNM)
• Three years of relevant experience in geriatric mental health
• Valid B.C. Driver’s License with vehicle access
