Key Responsibilities:
• Deliver primary health care services to patients
• Perform patient assessments and chronic disease management




• Educate patients on healthy lifestyle choices
• Collaborate effectively with healthcare team members
• Conduct follow-up care for patients

Requirements:
• Degree in Nursing from an accredited institution
• Registered Nurse license in Ontario
• At least one year in family practice or ambulatory care
• Certification in Basic Life Support (BLS) required
• WHMIS Certification (renew every two years)
