Key Responsibilities:
• Respond promptly to patient referrals
• Complete and document care assessments
• Assess risk to ensure safety for all
• Develop individualized care plans
• Lead staff training and performance evaluations
Requirements:
• Registration with College of Nurses of Nova Scotia
• At least 2 years of supervisory experience
• Excellent client and staff engagement skills
• Valid driver’s license in Nova Scotia
• Ability to work independently and within teams
