As part of a agile healthcare team, you'll execute care plans, report changes to the Registered Nurse, and create a secure atmosphere for patients during their transition back to the community.
Key Responsibilities:
• Follow KARDEX for patient care needs
• Execute individualized care plans and instructions
• Report patient condition changes to Registered Nurse
• Assist with psychosocial needs and respond to calls
• Document care and patient condition changes accurately
Requirements:
• High school diploma or equivalent
• Current HSCPOA registration
• Personal Support Worker certification required
• Minimum one year experience preferred
• Solid communication and teamwork skills
