You'll work alongside health professionals in a collaborative multidisciplinary environment, helping individuals achieve positive outcomes while developing your own skills and experience.
The Role Manage a caseload of adults with complex mental health needs.
Complete Care Act assessments, reviews and support planning.
Undertake Mental Capacity Assessments and Best Interest decisions where appropriate.
Work closely with NHS colleagues,



community mental health teams and partner agencies.
Carry out safeguarding enquiries and risk assessments.
Promote recovery, independence and strengths-based practice.
Maintain accurate and timely case recordings.
Requirements Social Work England registration.
Previous experience within an Adult Mental Health Team.
Sound knowledge of the Care Act 2014, Mental Capacity Act and Mental Health Act.
Excellent assessment, communication and partnership working skills.
Ability to manage a varied caseload in a rapid-paced environment.
