Become an Integrated Behavioral Health Specialist at Blackfoot Medical Pavilion, focused on delivering multidisciplinary behavioral health services in a primary care setting. Collaborate with the care team to ensure optimal patient outcomes across all ages.

Under the guidance of the Chronic Care Management Manager, your role includes providing direct social work and case management services to patients and families. You will complete psychosocial assessments and facilitate referrals for behavioral health support, crisis interventions, and short-term counseling. Excellent communication and organizational skills are essential for success in this full time position.

Key Responsibilities:
• Conduct psychosocial assessments for patients and families
• Provide crisis intervention and solution-focused therapies




• Collaborate with the clinical care team and coordinate referrals
• Advocate for patient rights and promote self-determination
• Document communications and interventions in EMR accurately

Requirements:
• Master’s Degree in Social Work required
• Licensed Specialist Clinical Social Worker (LSCSW) in Kansas or Missouri
• Minimum of one year’s experience preferred
• Knowledge of community behavioral health resources needed
• Ability to work independently and collaboratively
