Take on a pivotal role as a Transition Services Coordinator with Vancouver Coastal Health (VCH) at Vancouver General Hospital. You will manage the critical process of patient transitions from hospital to home or community care.

This position calls for an experienced nurse with a solid foundation in community health services and quality improvement practices. You will work closely with care teams and utilize evidence-based approaches to ensure smooth and protected patient discharges. Your responsibilities also include mentoring colleagues while facilitating clinical discussions to optimize care

Key Responsibilities:
• Coordinate discharge processes for transitioning patients
• Evaluate client needs to ensure suitable community resources




• Implement and monitor policies in the transition care program
• Consult and educate families about available services
• Manage documentation and client assessments accurately

Requirements:
• Valid RN registration with BCCNM
• Five years of nursing experience, including community settings
• Proficiency in clinical assessment tools
• Knowledge of evidence-based nursing practices
• Effective collaboration and communication skills
