Join the EOPCN as a Mental Health Social Worker focusing on connecting patients with essential resources and offering social work support. This in-person role is vital in supporting mental health through direct patient interaction and community resource connection.
The EOPCN is seeking an experienced social worker to empower patients within the mental health program. You will guide clients through their social worries while collaborating with a agile interdisciplinary team. Emphasis will be on holistic care, addressing everything from housing to emotional support for individuals in crisis.
Key Responsibilities:
• Interview patients and families to gather essential background information
• Assess social determinants and create culturally appropriate care plans
• Co-facilitate workshops to enhance biopsychosocial health
• Advocate for essential needs like food and housing assistance
• Document patient progress accurately for care continuity
Requirements:
• Minimum two years of social work experience
• Diploma or Bachelor’s degree in Social Work
• Active registration with Alberta College of Social Workers
• Solid local resource knowledge in mental health
• Proficient with Microsoft Office and EMRs
