As a key member of the team, you will conduct independent assessments, develop tailored care plans, and implement follow-up procedures. Collaborating with healthcare specialists, you will ensure clients receive top-notch management of their chronic diseases. You will also deliver health education sessions and maintain accurate client records, ensuring communication flows smoothly within the care team.
Key Responsibilities:
• Assess and monitor clients' health to achieve optimal outcomes
• Educate clients on chronic disease management and prevention
• Collaborate with primary care physicians and team members
• Maintain thorough client records and documentation
• Develop educational materials for various public audiences
Requirements:
• Bachelor’s degree in Nutritional Science
• Completed accredited Dietetic Internship
• Registration with the College of Dietitians of Prince Edward Island
• Experience in clinical dietetics
• Robust communication and computer skills
