We are seeking a licensed Registered Practical Nurse to join our South West program team. The role requires conducting assessments and providing comprehensive care through treatment plans, coordination, and rehabilitation support. Your expertise will empower clients and contribute to their recovery journey.

Key Responsibilities:
• Conduct mental and physical health assessments for clients
• Develop and coordinate individualized treatment plans
• Monitor client progress and connect with community resources




• Collaborate with psychiatrist for follow-up care
• Assist in developing individual empowerment plans

Requirements:
• Licensed Registered Practical Nurse with a positive standing
• Minimum four years of experience in mental health settings
• Excellent health assessment and clinical judgment skills
• Knowledge of relevant mental health legislation
• Valid Canadian driver’s license and liability insurance
