This position offers the opportunity to work weekdays and alternating weekends with a morning shift from 8 AM to 4 PM or an evening shift from 2 PM to 10 PM. You will engage with patients using cooperative care plans, assessing their needs while building long-term therapeutic relationships. Key Responsibilities:
• Assess patients’ individualized needs using appropriate tools
• Execute and monitor care plans continuously
• Engage patients collaboratively to build trust




• Maintain and update qualified patient records
• Provide on-call coverage to ensure patient safety
Requirements:
• Registration with CNO as Registered Practical Nurse in valuable standing
• Valid driver’s license and access to a vehicle
• Community care experience is an asset
• Reliability and flexibility are essential
• Willingness to travel to patient residences
