Key Responsibilities:
• Provide resident-centered care aligned with care plans
• Accurately document care assessments and outcomes
• Ensure a protected setting by adhering to safety guidelines
• Actively engage in Interdisciplinary Team meetings
• Supervise Health Care Aides and manage responsibilities
Requirements:
• Graduate from a recognized Licensed Practical Nursing program
• Active Registration with the College of LPNs and HCAs
• Strong communication and critical thinking skills
• Current CPR certification
• Experience with dementia care preferred
