Key Responsibilities:
• Provide responsive healthcare amid acute and chronic conditions
• Empower participants through education on health management
• Administer medications and conduct thorough health assessments




• Facilitate individualized care plans with ongoing participant engagement
• Collaborate with multidisciplinary teams and community resources
Requirements:
• Active Licensed Practical Nurse registration with BCCNM
• 1 to 5 years’ relevant nursing experience in complex health issues
• Experience in harm reduction and community health initiatives
• Dedication to combating stigma and promoting equity
• Proficient in both verbal and written communication
