Key Responsibilities:
• Offer holistic inpatient and community-focused care
• Lead client assessments and care plan development
• Facilitate team communication and planning sessions
• Assist clients in building effective self-care strategies
• Connect clients with essential community support services
Requirements:
• Registered with the College of Nurses of Ontario
• Minimum of two years nursing experience required
• Experience in community or home health care preferred
• Robust clinical assessment and leadership skills
• Valid driver’s license and the ability to travel is necessary
