You will undertake short-term counselling and facilitate essential community programs while utilizing a social service framework.
Key Responsibilities:
• Provide short-term counselling to patients and client groups
• Connect individuals with local community resources
• Collaborate with healthcare team members for assessments




• Facilitate group programming for social support
• Monitor and evaluate service delivery metrics
Requirements:
• Diploma in Social Service Work from an accredited college
• Registered with Ontario College of Social Workers and Social Service Workers
• Clinical experience in primary care settings preferred
• Proficiency in Rapid Assessment models
• Must be available for evening hours
