As an essential member of the interdisciplinary Family Health Team, the Nurse Practitioner will provide primary health care services across the lifespan. You will focus on health promotion and collaborate with physicians and other health professionals. This role includes prospects for chronic disease management and acute care across diverse patient populations.
Key Responsibilities:
• Provide comprehensive patient assessments and follow-up care
• Prescribe medications and order diagnostic tests




• Support chronic disease prevention initiatives
• Perform physical exams and immunizations
• Facilitate referrals and navigate the health system
Requirements:
• Current RN-EC registration with CNO
• Primary Care Nurse Practitioner Certificate
• Minimum 2 years in primary care or community health
• Excellent communication and assessment skills
• Flexibility for evening and Saturday shifts
