The salary range for this position is CAD $54.49 - $68.10 per hour.

Job Summary
Relief full‑time Clinical Pharmacist position at Abbotsford Regional Hospital and Mission Memorial in Abbotsford and Mission, BC. Available until approximately September 2027 or until the return of the incumbent.

Clinical Services

Individualize patient medication therapy

Counsel patients about medication therapy

Collaborate with physicians and other health care providers to develop, implement, and monitor therapeutic plans

Assist in developing drug therapy guidelines for individual disease conditions

Prepare and submit case studies and articles for publication in qualified journals

Act as department resource person within assigned area

Participate in medication use evaluations, quality improvement initiatives, and clinical drug trials

Provide mentorship and supervision to pharmacy students, residents, and new staff

Validate prescriptions, dispense accurately, and resolve safety concerns

Qualifications

Current registration as a Pharmacist with the College of Pharmacists of British Columbia

Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience

Broad knowledge of organization policies, procedures, and standards of care

Knowledge of evidence‑based clinical pharmacy practice, medication use evaluation, research processes and methodology, and drug distribution processes in a hospital setting

Ability to apply clinical pharmacy knowledge and make decisions with minimum supervision

Excellent communication skills, both verbal and written

Strong organizational and prioritization skills

Ability to establish and maintain effective working relationships with health practitioners, patients, families, and the public

Physical ability to carry out the duties of the position

Responsibilities

Consult with physicians, nurses,



and other health care providers to develop and discuss therapeutic plans based on chart review and assessment of clinical responses; ensure appropriate documentation in the patient health record.

Individualize patient medication therapy by reviewing literature, patient charts, lab results, and meeting with patients and the health care team.

Provide medication counselling, explain procedures, techniques, and equipment, and assess patient/family understanding.

Assist with developing drug therapy guidelines and monitor effectiveness of current guidelines.

Participate in ward rounds, program team meetings, and unit‑level pharmacy services.

Act as department resource person, support staff with drug information on disease states.

Contribute case studies and review articles to professional journals.

Provide instruction and supervision to pharmacy students and residents.

Participate in quality improvement activities, audits, and recommendations.

Perform drug distribution duties, interpret medication orders, dispense medications using a computerized medication information system, and ensure accuracy and compliance with policies.

Review prescriptions for therapeutic suitability and clarify with prescribers as needed.

Maintain accurate dispensing records, including those for narcotic and controlled drugs.

Prepare reports on activities, maintain inventories, monitor expiry dates, and report errors.

Participate in clinical drug trials, maintain documentation, and review drug adverse reactions.

Implement medication safety initiatives and quality control activities.

Participate in staff meetings, committees, and other programs as required.

Perform other related duties as assigned.
