The Canadian Mental Health Association (CMHA-S/M) is hiring a full time Case Manager for its Residential Addiction Treatment Program in Sudbury. This role focuses on delivering holistic and trauma-informed care.
As a Case Manager, you will provide therapeutic support to individuals in a residential program, guiding them through personalized treatment plans. Your responsibilities will include conducting assessments, delivering group counseling, and working closely with other professionals. You will demonstrate robust communication skills and a commitment to person-centered approaches, ensuring the best outcomes for residents.
Key Responsibilities:
• Provide therapeutic support and counseling to residents
• Coordinate with multidisciplinary teams for care planning
• Facilitate individualized and group counseling sessions
• Conduct assessments to tailor treatment plans
• Support residents through their recovery journeys
Requirements:
• Degree or diploma in health or social services
• At least 1 year of case management experience
• Registration with a skilled body preferred
• Computer proficiency and knowledge of assessment software
• French conversational skills required for some positions
