Join Haven Toronto as a Support Care Worker in the Hospital Overflow Program. This full time role focuses on client-centered support for individuals facing homelessness and mental health challenges.
As part of a dedicated support team, the Support Care Worker will work overnight shifts to assist clients in a protected and welcoming environment. Key responsibilities include orienting new clients, conducting assessments, and providing direct support or referrals. You will also maintain respectful and supportive relationships, helping clients access essential services and resources.
Key Responsibilities:
• Welcome and register current clients into the program
• Conduct assessments of health and well-being
• Assist with appointment scheduling and follow-ups
• Document client interactions and needs accurately
• Monitor safety and provide crisis intervention as necessary
Requirements:
• Post-secondary diploma in Social Services or related field
• Strong knowledge of issues related to homelessness
• First Aid and CPR certifications
• Experience in a trauma-informed care setting
• Excellent communication and interpersonal skills
