In this full time permanent position with Peterborough Regional Health Centre, you will serve clients facing homelessness and mental health or addiction issues. Your work will focus on assessment, rehabilitation, and recovery interventions while ensuring a trauma-informed approach. The Integrated Health Team will rely on your expertise to develop personalized treatment plans and navigate the healthcare system effectively.
Key Responsibilities:
• Assess client mental health and functional needs
• Create and execute individualized rehabilitation plans




• Provide integral case management and program facilitation
• Work collaboratively with a multidisciplinary team
• Advocate for clients’ needs and housing support
Requirements:
• B.Sc. in Occupational Therapy or equivalent
• Registration with the College of Occupational Therapists of Ontario (COTO)
• Minimum of three years’ relevant experience preferred
• Effective problem-solving and decision-making skills
• Ability to transport clients as required
