In this capacity, you will report to the Program Manager and engage with various stakeholders to support individuals with mental and physical disabilities. You will be responsible for conducting intake procedures and developing personalized plans. Continually working with clients ensures that their basic needs and quality of care are prioritized.
Key Responsibilities:
• Conduct intakes and assessments for recent guests
• Develop individualized service plans for tenants
• Facilitate daily living assistance including personal grooming




• Maintain client documentation and evaluative metrics
• Actively participate in health and safety compliance
Requirements:
• Diploma in Community Social Services or similar
• At least two years of relevant experience
• Robust problem-solving and organizational skills
• Valid Class 5 driver’s license and insurance
• Criminal Record Clearance for the Vulnerable Sector
