In this vital role, you will implement individualized support plans that cater to the unique goals of Service Users and their families. Your work will ensure a high quality of life and promote greater independence. Responsibilities include offering personal care, organizing transportation, and facilitating involvement in community activities.
Key Responsibilities:
• Deliver personalized outcome-based support to Service Users
• Help with personal care needs and mobility assistance
• Assist in goal-setting and tracking progress
• Use case management software for daily activity records
• Research community services that match Service User plans
Requirements:
• Completed two-year post-secondary program in related field
• Current First Aid and CPI certifications
• At least two years’ experience in social services
• Valid Ontario Class “G” Driver’s License and vehicle
• Solid judgment and communication skills
