Join the YMCA as a Housing First Support Worker to assist youth in transitioning to secure housing. This full time role requires expertise in case management and community reintegration.
Your work will center on helping homeless youth find permanent housing supports while facilitating the development of essential life skills. Through proactive engagement and collaboration with various community resources, you will enhance the stability and independence of your clients. Your role includes engaging clients in goal-setting and ensuring they receive necessary healthcare resources.
Key Responsibilities:
• Actively engage with clients in their communities
• Assist with referrals to medical and financial supports
• Develop personalized case management plans for clients
• Ensure regular assessments and monitoring of client needs
• Facilitate connections with social and recreational activities
Requirements:
• Two years’ experience in social or human services
• Knowledge of mental health and harm reduction strategies
• Certification in crisis prevention techniques
• Valid First Aid/CPR Certification
• Awareness of diversity and inclusion principles
