The Weeneebayko Area Health Authorityis seeking an experienced professional to fill the key role of Patient Navigator. Reporting to the Clinical Manager, you will assist patients in navigating the WAHA, provincial, and federal health systems.

What You’ll Do
Work in collaboration with the Healthcare Team to plan an effective discharge and community care process to ensure an easier transition back into the home environment.
Act as a resource and facilitator to discharge planning team, physicians, and staff.
Assist in the development of care plans when clients are back in their home settings.
Monitor clients progress while they are back in their home settings to determine if needs change over time.
What You Bring
Ability to communicate in the local native Cree language is an asset
Diploma as a Social Service Worker is required
Bachelor of Social Work is strongly preferred
Registration in positive standing with the Ontario College of Social Workers or a willingness to obtain is required
Relevant experience in discharge planningSolid networking and outreach skills
Understanding of patient centred philosophy
Experience working with the senior’s population an asset
Must possess a valid driver’s license for Ontario
