Indigenous Patient Navigator – Vancouver Coastal Health (VCH)

About the Role

Vancouver Coastal Health is looking for an Indigenous Patient Navigator to join the Indigenous Health Patient Experience team. The role supports culturally safe navigation for First Nations, Metis, and Inuit clients and their families.

Key Responsibilities

- Provide day‑to‑day health system navigation support, creating safe people, places, and systems for First Nations, Metis, and Inuit clients and families.
- Liaise with clients and primary care teams to ensure successful access to primary care and community‑based health care services.
- Ensure patient care occurs in a culturally safe and respectful manner by connecting with clients and families, helping them voice concerns, offering traditional culturally safe resources, and mitigating potential complaints through communication and understanding.
- Liaise with Indigenous community‑based agencies.
- Inform and influence program development, planning, and evaluation of services for First Nations, Metis, and Inuit clients through the patient experience process of navigating the healthcare system.

Qualifications and Experience

- 2 to 3 years’ recent experience working with Indigenous communities in British Columbia.
- Bachelor’s degree in Health Care, Social Work, Nursing or related discipline preferred.
- Travel within the health authority’s geographic area is required, with access to appropriate transportation.
- Demonstrated knowledge of Indigenous health and culturally safe care, including completion of Indigenous Cultural Safety Training.
- Demonstrated understanding of First Nations, Inuit, and Metis Peoples and history.
- Knowledge of provincial and federal guiding principles (e.g.,



In Plain Sight report, Calls to Action).
- Ability to identify health‑care system gaps and power inequities related to colonialism.
- Knowledge of Indigenous and non‑Indigenous services and resources within VCH communities of care.
- Experience applying Trauma‑Informed Practice, Harm‑Reduction, and Strengths‑Based care.
- Understanding of mental health and substance use issues.
- Interpersonal and communication skills, both verbal and written.
- Intermediate skill with word processing, spreadsheets, and database programs.
- Physical ability to perform the duties of the position.

Preferred Background

Strong preference given to candidates identifying as Indigenous (First Nations, Métis, or Inuit).
