We are seeking a caring and compassionate Nurse Practitioner to provide primary health care within the Ontario Ministry of the Solicitor General’s correctional facilities. The role focuses on health promotion, chronic disease management, pain and symptom management, and mental health and addictions care.

Responsibilities

- Assess, diagnose and prescribe treatment with an emphasis on health promotion and disease prevention
- Communicate diagnosis and treatment plan options to patients, staff and external stakeholders
- Collaborate with an inter‑professional team to develop the patient plan of care and implement health promotion strategies
- Advocate for and facilitate self‑care principles, involving offenders in their own plan of care
- Act as a resource for patients, staff and external stakeholders
- Respond to emergency situations and implement appropriate interventions
- Practice in accordance with College of Nurses of Ontario Practice Standards and Ministry expectations
- Practice in accordance with the security requirements of the institution

Mandatory Requirements

- Registered as a Registered Nurse Extended Class (Nurse Practitioner) in good standing with the College of Nurses of Ontario
- Holds a Nurse Practitioner specialty certificate in primary health care or adult care

Clinical Assessment & Decision‑Making

- Demonstrate clinical expertise and assessment skills to elicit, interpret and utilize information from patients and other sources
- Respond to emergency situations and implement appropriate interventions
- Evaluate responses to treatment and modify the treatment plan to achieve established goals

Leadership & Quality Improvement

- Coordinate and deliver primary health care services, embed quality improvement, and lead consultation and engagement with healthcare staff and partners, as well as internal and external stakeholders

Specialized Knowledge

- Knowledge of illness/disease theory, pharmaceuticals, diagnostic and laboratory testing, evidence‑based practice and current treatment modalities




- Knowledge of mental health and addiction treatment options
- Interpret and apply legislation to practice (e.g., Health Care Consent Act, Personal Health Information Protection Act, Regulated Health Professions Act)

Communication & Interpersonal Skills

- Communicate effectively and present information in a clear and concise manner
- Document care and progress by preparing progress notes, written treatment plans and clinical summaries
- Adapt communication skills to provide information and health teaching to a diverse population
- Advocate for and encourage patient participation in their plan of care
- Support patients in the implementation and modification of their treatment plan

Hours of Work

36.25 hours per week. Rotating shifts in a health care unit that provides 7 days per week service, up to 24 hours a day.
