Nurse role focuses on delivering compassionate end-of-life care in clients' homes. We are seeking a dedicated Registered Nurse for our Palliative Care program in Central Ontario. Providing client-centered nursing services, you'll collaborate with a diverse team of healthcare professionals.

Key responsibilities include assessing, monitoring, and coordinating care for individuals and families, ensuring high-quality patient care through ongoing evaluations. Key Responsibilities:

- Assess and monitor patient conditions regularly
- Coordinate care planning and ensure effective interventions
- Evaluate care plans with continuous reassessment
- Maintain documentation standards as per College of Nurses
- Participate in on-call rotations for client needs Requirements:
- Registered with the College of Nurses of Ontario
- RPN diploma or RN degree from a recognized institution
- Valid First Aid or CPR Certification
- Experience in acute care or community nursing
