Key Responsibilities

- Provide leadership, coaching, and mentorship.

- Collaborate with pharmacy staff to ensure productive, accurate, and timely medication distribution.

- Lead or support continuous quality improvement initiatives aligned with the Ontario College of Pharmacists Hospital Pharmacy Assessment Criteria and Accreditation Canada standards.

- Uphold organizational values, strategic goals, and professional Standards of Practice (OCP, NAPRA, CSHP).

- Participate in committees, projects, policy/process development, and research activities.

- Support medication management activities, including medication reconciliation, care planning, safety initiatives, interdisciplinary rounds, and resolving drug-related issues.

- Work collaboratively with healthcare professionals and stakeholders to optimize patient care.

- Precept pharmacy learners (students, residents).

- Contribute to formulary management and drug use evaluations.

- Provide flexible pharmaceutical care across clinical program areas
- Work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the Royal.

Qualifications

- Bachelor of Science in Pharmacy or entry level Doctor of Pharmacy (PharmD), from a recognized university as determined by the Ontario College of Pharmacists (OCP).

- Completion of a hospital pharmacy residency, or a postgraduate clinical pharmacy degree is considered an asset.

- Minimum of two years of experience as a hospital pharmacist is considered an asset.

- Registration and in good standing with the Ontario College of Pharmacists (Part A).

- Experience or familiarity in CPOE (computer prescriber order entry), BMV (bedside medication verification, eMedRec (electronic medication reconciliation),



eMAR (electronic medication administration records), EMR (electronic medical/ health record) is considered an asset.

- Experience working with electronic health record clinical software applications
- Experience in the medication reconciliation process.

- Working knowledge of modern hospital care and drug therapy, including knowledge of medication safety risks in health care.

- Experience or familiarity in pharmaceutical research in health care settings.

- Excellent interpersonal skills.

- Ability for self-directed learning and working.

- Excellent written and verbal communication and organizational skills.

Language Requirements

English level A- is mandatory in oral expression, comprehension, reading and writing.

Bilingual (French/English) is considered an asset.
