Duties

- Manage patient care plans, coordinate referrals to specialists, and follow up on treatment progress.
- Collect and analyze health data to monitor patient outcomes and improve care strategies.
- Utilize electronic health record systems such as Epic and Cerner for documentation, order entry, and data management.
- Provide pediatric care when necessary, addressing the unique needs of children and their families.
- Support home care initiatives by developing treatment plans suitable for patients receiving care outside the clinical setting.




- Participate in research activities or quality improvement projects to enhance clinical practices.
- Collaborate with healthcare teams to ensure seamless patient care delivery across various settings.

Experience

- Proven hospital experience providing general medical services in fast-paced environments.
- Strong background in acute care settings with the ability to handle urgent medical situations confidently.
- Experience in pain management techniques and strategies for diverse patient populations.
- Familiarity with pediatric medicine, including preventive health measures for children.
- Prior medical office experience demonstrating proficiency in patient care workflows and documentation systems like Epic or Cerner.
- Skilled in data collection methods relevant to clinical research or quality assurance initiatives.
- Knowledge of medication administration protocols adhering to safety standards.
- Experience supporting home care programs or outpatient services is highly desirable.

Pay: $170,000.00-$220,000.00 per year
