As a full time Psychogeriatric Resource Consultant, you will work with the Seniors Mental Health Behavioral Support Services team at Providence Care. This role involves supporting Long Term Care, Acute Care, and community agencies by enhancing staff capacity and developing educational initiatives for personnel working with older adults facing complex health issues. Your expertise will drive the development of integrated care systems, aiming to improve outcomes for clients and their caregivers.
Key Responsibilities:
• Provide case-based consultations to healthcare providers
• Develop relationships with community healthcare partners




• Identify community learning needs collaboratively
• Participate as an integral member of the SMH BSS team
• Support implementation of service delivery models
Requirements:
• Degree in a Health Care Discipline; Master’s preferred
• Minimum three years in community healthcare
• Expertise in seniors mental health assessment
• Experience with PIECES framework and GPA
• Proven educational intervention development skills
