In your role with the Wound Ostomy Continence (WOC) Team, you'll conduct thorough assessments and implement care plans for those with wound and continence issues. Collaboration with other healthcare professionals is key, along with a commitment to enhancing community health outcomes. SE Health promotes ongoing education and leadership support.
Key Responsibilities:
• Conduct holistic assessments for clients with continence issues
• Formulate and monitor treatment plans with team input
• Provide mentorship and educational sessions on wound care
• Engage in quality monitoring for optimal health results
• Support research initiatives within the organization
Requirements:
• Completion of NSWOCC WOC-EP program needed
• Current RN status in Ontario
• Strong commitment to quality care
• Willingness to travel, valid driver’s license required
• Knowledge in community health care preferred
