Join the Wound Ostomy Continence (WOC) team at SE Health in this key nursing position. As a Skin Wellness Associate Nurse, you will conduct assessments, develop care plans, and educate others in wound and ostomy management. Working closely with the Nurse Specialized in Wound, Ostomy and Continence (NSWOC), your contributions will enhance client care.
Key Responsibilities:
• Conduct detailed assessments of clients' needs
• Formulate treatment plans with the interprofessional team
• Evaluate care plans and escalate when necessary




• Collaborate with K2P Wound Care Resource Nurses
• Provide mentorship and educational guidance
Requirements:
• Current registration with the College of Nurses of Ontario
• Completed SWAN program by the Wound, Ostomy and Continence Institute
• Excellent communication and critical thinking skills
• Valid driver's license and vehicle for travel
• Must be willing to work versatile shifts
