What You’ll Be Doing Conduct comprehensive nutrition assessments for patients. Develop and implement individualized nutrition care plans. Provide counseling and education and in‑services to patients, using a trauma‑informed and patient‑centered approach. Document as per HHC and college standards, including assessments, interventions, and outcomes in patient medical record. Collaborate with the clinical team to provide the best nutritional care for patients. Advise and develop custom menus, snacks. Promote healthy eating behaviors. What We’re Looking For Bachelor’s degree in Human Nutrition and Dietetics from an accredited post‑secondary institution.



Valid registration with the College of Dietitians of Ontario is required; membership in Dietitians of Canada is preferred. 5 years’ previous experience in Older Adult and Eating Disorders. Experience with mental health and addictions populations. Collaborative, service‑oriented, innovative and accountable. Commitment to working within Homewood’s Health and Safety Standards. Practice in accordance with the ethical guidelines of the Dietetics profession.
