Join our mission to empower homeless youth in Ottawa as a Youth Housing Based Case Manager. Provide culturally competent care while emphasizing a holistic, feminist approach to case management.
In this role, you'll be responsible for case management of ten homeless youth, ages 16-24, ensuring they receive the necessary support in navigating various challenges. You will actively collaborate with Elders and community members while maintaining qualified boundaries and a non-judgmental mindset. Your independence and advocacy skills will be essential as you manage client files and progress reviews efficiently.
Key Responsibilities:
• Collaborate with Elders and team members for cultural competency
• Facilitate assessments and support plans for youth
• Maintain confidential client relationships with respect
• Organize and evaluate case documentation
• Assist clients in accessing necessary services
Requirements:
• Diploma or relevant experience in social services
• 3 years of case management or advocacy experience
• Knowledge of Indigenous and non-Indigenous services in Ottawa
• Strong organizational and time management skills
• Experience providing service to youth with complex needs
