As a Mental Health Case Manager, you will engage with clients aged sixteen and over who experience mental health issues, dual diagnosis, or addictions. Your work will encompass direct service interventions and clinical support, aimed at enhancing the quality of life for those who are vulnerably housed or homeless. The role requires daily travel within York region and a collaborative approach to case management.
Key Responsibilities:
• Provide intensive case management and advocacy services

Requirements:
• Bachelor’s degree in social work or related field
