Join the HART Hub team in Woodstock, Ontario, as an Addiction and Mental Health Specialist. This full time role focuses on providing compassionate support and integrated care for individuals facing substance use challenges.
In this position at the Oxford Wellness Centre, you will lead intake and assessment processes, guide clients through their recovery journey, and ensure their essential needs are met. You'll collaborate with community partners for holistic client care while maintaining an understanding of best practices in substance use treatment.
Key Responsibilities:
• Administer screening tools and conduct eligibility assessments
• Develop integrated care plans addressing mental health and addictions




• Provide onsite support for client necessities and well-being
• Engage in proactive crisis intervention strategies
• Facilitate community conferences and discussions on client care
Requirements:
• University degree or diploma in a related field
• Satisfactory Police Vulnerable Sector Check required
• Valid G Driver's license and vehicle with insurance
• Experience in community-based addiction support
• Regular attendance at work is essential
