You will assess healthcare needs, manage disease risks, and educate patients on health issues. A focus on developing tailored patient care plans in accordance with current disease management protocols is also critical. Key Responsibilities:
Conduct face-to-face and phone consultations efficiently
Handle queries and medical paperwork accurately

Maintain detailed consultation records for audits

Requirements:
Fully licensed GP with GMC registration
On the NHS England medical performers list
Proficient in chronic disease management
Committed to continual learning and development
Excellent communication skills with patients
