Enhance community health as a Part time Physician with the Health Centre, dedicated to outreach services in Kitchener. Provide care for homeless individuals and those with complex needs. As a part time physician working 22.5 to 30 hours weekly, you will be integral to outreach teams addressing the needs of vulnerable populations.

This role will involve collaboration within an interdisciplinary team to improve health outcomes and deliver comprehensive care tailored to individual situations. Key Responsibilities:
Provide comprehensive medical assessments and treatments
Address community health using a Social Determinants approach
Evaluate addiction and mental health issues directly
Facilitate triage for client needs in various settings
Collaborate on implementing public health initiatives

Requirements:
Medical degree and current CPSO license
Minimum three years of experience in community healthcare
Experience with complex population needs
Skilled in Microsoft Office and medical records systems
Valid driver’s license and access to a vehicle
