Become a key part of Humber River Health as a Registered Nurse in the Home Hemodialysis Long-Term Care Satellites. Deliver vital care and support for chronic kidney disease patients. This permanent part-time position involves direct patient interactions, effective treatment coordination, and collaboration with a agile team. You will champion evidence-based clinical practice and support patients through independent dialysis education and care management. Key Responsibilities:
Coordinate clinical activities for dialysis patients
Conduct assessments and provide educational support
Facilitate communication among care team members
Participate in quality improvement initiatives
Document clinical data accurately for reporting Requirements:
Registration in valuable standing with the College of Nurses of Ontario
At least 2 years of nephrology nursing experience
Competency in clinical documentation systems
BLS certified, CNeph certification preferred
