Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a solid foundation in nursing practices, excellent communication skills, and the ability to adapt to diverse patient needs.
The LPN will work closely with multidisciplinary teams to ensure optimal health outcomes for patients of all ages. Level 3g & 3B. Position could lead to managing.
Duties
Administer medications and treatments accurately according to physician orders and established protocols
Monitor patient vital signs and observe for changes in condition
Assist with patient assessments and document findings using electronic health records such as Epic or Cerner systems
Provide wound care, including dressing changes and wound assessments
Support patients with activities of daily living and ensure their comfort and safety
Collaborate with physicians, nurses, and other healthcare specialists to develop and implement care plans
Educate patients and families about health management, medication use, and post-discharge care instructions
Maintain accurate documentation of patient care activities in compliance with healthcare regulations
Participate in case management activities to coordinate comprehensive patient services across different settings
Requirements
Valid state licensure as a Licensed Practical Nurse (LPN) or equivalent certification
Prior experience in nursing home, hospital, acute care, dementia care, pediatrics, home care, or hospice settings is highly preferred
Knowledge of medical terminology, medication administration protocols, and pain management techniques
Experience working with electronic health records such as Epic or Cerner is desirable
Solid patient care skills with a focus on senior care, assisted living, or working with children is advantageous
Laboratory experience including phlebotomy is a plus
Excellent communication skills and the ability to work effectively within a multidisciplinary team
Ability to handle multiple responsibilities with compassion and professionalism
Commitment to maintaining patient safety standards and adhering to healthcare regulations
Job Type: Permanent Pay: $22.00-$25.00 per hour
