Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care settings. This role requires a strong understanding of medical procedures, excellent communication skills, and a commitment to improving patient outcomes. The LPN will work closely with interdisciplinary teams to deliver personalized care tailored to each patient's needs.

Duties
Administer medications and treatments accurately according to physician orders and established protocols
Monitor patient vital signs and assess patient conditions regularly
Assist with patient admissions, discharges, and transfers
Document patient care activities using electronic health records such as Epic or Cerner systems
Provide wound care, including dressing changes and wound assessments
Support patients with activities of daily living and mobility assistance
Conduct phlebotomy procedures and laboratory specimen collection when necessary




Educate patients and families about health management, medication use, and post-discharge care plans
Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop comprehensive care plans
Ensure compliance with all safety standards, infection control protocols, and regulatory requirements

Experience
Prior experience in nursing home, hospital, or acute care settings preferred
Very positive knowledge of use the PCC system to make care plan
Experience in dementia care, senior care, hospice care, or pediatrics is highly desirable
Knowledge of medical terminology and medication administration procedures required
Background in home care or assisted living environments is a plus
Experience with pain management, case management, or laboratory procedures enhances candidacy
Solid patient care skills combined with excellent communication abilities are essential for success in this role

Job Types: Full-time, Part-time, Permanent

Pay: $34.00-$42.08 per hour
