A part time (0.2 FTE) Family Practitioner (FP) role is available to deliver outreach services in Sooke, B.C. at the Sooke Food Bank and Sooke Shelter.
Services will be provided to adults and children who are impacted by poverty, colonization, stigma, and homelessness.
Providing accessible primary episodic care services that not only address immediate medical needs but also actively work towards decreasing barriers and social determinants of health in accessing care.
The FP will be supported by a primary care RN, an MHSU RN and a MOA and provide both OAT and primary care services to a broad range of clients.
