Position Overview Shared Health leads the planning and coordinates the integration of patient- centered clinical and preventive health services across Manitoba.

POSITION OVERVIEW Under the general direction of the Manager of Health Services, the Indigenous Liaison Coordinator works with renal patients to facilitate care transitions between dialysis modalities and other Kidney Health services.





The Indigenous Liaison Coordinator takes a home first approach to renal care and works with patients, their families, and the interdisciplinary Kidney Health team to overcome barriers to home dialysis.

Using a patient centred approach and with a focus on Indigenous patients across the province, this role provides education to those with advanced kidney disease nearing the need for or on dialysis modalities to support informed decision making.

This role will also support transitions of individuals seeking to return to their home communities. THIS IS A DESIGNATED INDIGENOUS COMPETITION, OPEN TO INDIGENOUS APPLICANTS.
