As a vital member of a multidisciplinary team, you will deliver life skills training, coaching, and advocacy to clients and their families dealing with severe substance use, mental health, and behavioral issues. Key Responsibilities:
• Provide life skills training and coaching for clients
• Offer advocacy and support services in compliance with care plans
• Report on clients' progress and medication reactions
• Foster relationships promoting positive change and independence
• Collaborate with a multidisciplinary team for optimal care
Requirements:
• Diploma in Community Social Services or related field
• Minimum two years experience with Indigenous communities
• Knowledge of Indigenous health issues and harm reduction
• Valid Class 5 vehicle license
• Basic Life Saving Certificate (Level C)
