• Deliver personal care for daily living needs
• Document and report clinical data to RNs

• Communicate responsibly with patients and caregivers
• Ensure coordination of necessary home support resources
Requirements:
• PSW certification required to apply
• Experience as a PSW for 1–2 years preferred
• Certifications in CPR and First Aid mandatory
• Familiarity with fundamental Palliative Care practices
• Must have reliable vehicle access for travel
